- There will be five grand prizes of $1,000 Walmart gift cards, each awarded to a different winner.

Prize Details for Walmart’s Customer Survey
Walmart customers will receive two types of prizes:
- Five (5) grand prizes of $1,000 Walmart Gift Cards (the “Grand Prizes”) will be awarded. The approximate retail value of each Grand Prize is $5,000.
- Each participant will receive a $100 Walmart gift card as the first prize. The first prize has an ARV of $75,000 for all 750 participants.
- There is a total ARV of $80,000 for all prizes.
The Grand Prize
- Potential Grand Prize winners will be contacted by email or phone by the Independent Administrator by November 7, 2022 (the “Notification”). Winners must respond to the Independent Administrator’s Notification within five business days to claim the Grand Prize.
- If the Potential Prize Winner does not respond to the Notification within five business days of the Independent Administrator’s first Notification or if the Sponsor and Independent Administrator, in their sole discretion, determine that they are ineligible, an alternate Prize Winner will be selected from the remaining non-winning entries.
- From the date of Notification, an Affidavit of Eligibility and Liability/Publicity Release, along with a W-9 tax form (the “Paperwork”), are required. Grand Prize winners who are minors (but at least 18 years old) must have their parents or legal guardians sign the Paperwork.
- After reviewing the Affidavit, the Grand Prize will be sent by Priority Mail through the United States Postal Service on behalf of the student.
- Those who may win the Grand Prize must return the Paperwork by the specified date and time. Potential Grand Prize winners will be disqualified if they fail to return the Paperwork by the deadline specified in the Notification, are deemed ineligible, or do not follow the Official Rules.
- If the Notification is returned as undeliverable, another potential Grand Prize winner will be selected from all remaining eligible entries.
- Taxes due to the value of the Grand Prize, including federal, state, and local taxes, are the sole responsibility of the Grand Prize winners.
- Upon awarding the Grand Prize to the Grand Prize winner, the Sponsor must notify the Internal Revenue Service of the prize’s value. A Form 1099 will be issued to the winner and the IRS.
- After signing the Paperwork, Grand Prize winners will receive their prize within four weeks.
The First Prize
In four to six weeks after the drawing, potential winners will receive their First Prize at the address on file with the Sponsor or the address they provided at the time of entry. If the prize is returned, the address is too short, too long for shipping, or for any other reason, the winner may be disqualified.
There will be a new First Prize winner selected from the remaining entries. If the alternate First Prize winner returns their First Prize, their address needs to be more complete for shipping, or if the prize cannot be delivered, they will be disqualified, and the First Prize will not be awarded.
Prizes that cannot be awarded will not be compensated.

Rules for the Walmart Sweepstakes
The sweepstakes are governed only by U.S. law. Residents of any other state or territory are not eligible to participate.
The “Sweepstakes Period” runs from July 30, 2022, at 12:01:01 a.m. Central Time (“CT”) through October 28, 2022, at 11:59:59 p.m. CT.
- Eligibility:
It is open to legal residents of the fifty (50) United States, the District of Columbia, or Puerto Rico who are at least eighteen (18) years old or of the age of majority in their state of residence. The age at entry is nineteen (19) years old in Alabama, Nebraska, and Mississippi, and eighteen (18) years old in other states”.
This offer is not valid outside of the United States, the District of Columbia, Puerto Rico, or where prohibited or taxed.
Officers, directors, managers, and employees of Walmart Inc., as well as its parents, subsidiaries, affiliates, and related companies, as well as their advertising and promotion agencies, consultants, and agents, as well as their immediate families and those living in their household, are ineligible. Any person involved in the development, production, execution, or distribution of these Sweepstakes, including American Sweepstakes & Promotions, Inc., is not eligible.
“Immediate family member” refers to the spouse, children, mother, father, sister, sister-in-law, brother, or brother-in-law. The sweepstakes can only be entered from one of the fifty states.

During the Sweepstakes Period, participants who purchase at a participating Walmart store will receive a survey invitation on their receipt.
To enter, you will need to fill out the online entry form and take the survey within one (1) week of receiving the survey invitation at survey.walmart.com (the “Purchase Website”).
Please click “Submit” once you have completed the survey. Entrants may be asked to provide their first and last names, street addresses, city, state, zip code, phone number, and email address.
If the cash register receipt date is October 21, 2022, or later, surveys must be completed within one (1) week of the cash register receipt date for one (1) entry into the sweepstakes. Please complete the entry form, check the boxes, and click “Submit”. Surveys must be conducted by 11:59:59pm CT on October 28, 2022, if the cash register receipt date is October 21, 2022, or later.
(ii) Survey Invitation:
As part of the Sweepstakes Period, you will receive an email survey invitation (“Invitation”) if you purchase (of any amount) from Walmart.com, Walmart.com/grocery, a Walmart store, or any Walmart mobile app or website during the Sweepstakes Period and click the unique survey URL (“Survey Website”) in the Invitation.
Click the “Submit” button once you have completed the survey. All answers must be submitted for them to be completed. Upon clicking “Continue,” entrants may be asked to provide their first name, last name, street address, city, state, zip code, phone number, and email address (if they have one) in the entry form. To be eligible for the prize, you must complete the entry form.
One (1) entry will be granted if you complete the survey within one (1) week of the invitation date or by 11:59:59 pm CT on October 28, 2022, if the invitation date is October 21, 2022, or later.
